Guardianship Order
An unlimited guardianship order for a dependent child will allow you to make changes to your benefit plans outside of Open Enrollment.
To make changes to any of the plans listed immediately below, you will need to complete 
the Change of Status form and 
the subsequent personalized benefit enrollment form. Step-by-step instructions appear on the respective plan pages below. You must make the change within 30 days of the guardianship order.

Note regarding unlimited guardianship:
A guardianship order must be reviewed by University Human
Resource Services to ensure it meets university requirements.
You may also want to consider making changes to the plans
listed below. Such changes do not require completion of a change of status form and can be done at any time.
Change Beneficiaries on Retirement and Insurance plans
- Change the beneficiaries on your base retirement and life insurance, as well as any other supplemental plans.
IU Tuition Benefit (formerly Tuition Benefit)
- Dependents of staff members are eligible for the tuition fee courtesy plan.
IU Tax Deferred Account (formerly IU Tax Deferred Annuity Plan)
- Begin or stop IU Tax Deferred Account.
