Any PDF form with a beside it is a typeable form. A typeable PDF form allows you to type directly onto the form before you print it. To move from field to field, press "Tab"; you are unable to type on any field requiring a signature (as indicated on the form). You cannot submit these forms on-line; rather, once you have completed the form, print it, sign it (or obtain the necessary signatures), and submit it to our office by fax or mail, or in person. Please remember to attach any required documentation.
