Office of International Admissions Entry Term Change Request Form

All fields are required.

International Deferrals: International students must fill out and submit this form before the start of the term for which you originally applied. Please follow the instructions below depending on your level.

International Freshman Applicants: Undergraduate freshman applications and application fees are valid for one year from the entry term indicated on your original application.

If you need to defer your entry term beyond one year, you must reapply. If you register for 12 or more credit hours at a postsecondary institution (such as another college or university), you must reapply as a transfer student.

You are required to submit your final secondary school transcript and any other required information for the requested term if you have not done so already. Your application may be re-reviewed based on any new academic information. You must submit all required documents before the deadline for the term you wish to enroll at IU. A list of required documents can be found here.

International Transfer Applicants: Undergraduate transfer applications and application fees are only valid for one semester following the entry term indicated on your original application (excluding summer terms).

For any deferral period longer than one term, you must submit a new application and new application fee.

You must submit all required documents before the deadline for the term you intend to enroll. A list of required documents can be found here.

International Graduate Applicants: You may defer enrollment for two years from the entry term indicated on your original application. Your application fee will be valid for those two years.

Graduate-level deferral after admission requires the approval of the program.

If you have not been admitted to your desired program, or if you withdrew your application before you were admitted, you may defer your application to that program for one year from the entry term listed on the original application. Your application fee will be valid for one deferral of one year.

Regardless of whether or not you have been admitted to the program, we will require updated academic records, an outline of your activities from the time of your current application until the time your new desired term begins, and/or updated financial documents before the deadline for the term in which you intend to enroll.

Please reconsider my application for the following semester:

Do you plan to take classes, attend a school, or college or university before enrolling at IU? If yes, when do you plan to take these classes and at what institution?:

Since the submission of your original application, have you ever been charged with or convicted of a misdemeanor or felony? Have you engaged in behavior that resulted in mental or physical injury to person(s) or personal property?
Yes No

If yes to either question, please send the documentation specified in the paragraph below. Additional information may be requested and additional time may be required to review this information.

Criminal Activity Disclosure:

We are committed to maintaining a safe learning environment. As part of that commitment, we require applicants who have been charged with or convicted of a misdemeanor or a felony, or who have engaged in behavior that resulted in mental or physical injury to person(s) or personal property (including for example, but not limited to, behavior that led to a restraining order against you), to disclose that information. A previous conviction or previous conduct of the sort identified here would not automatically preclude admission to IU, but does require review. You must provide a brief explanation, including location (city, state, and country) of conviction or problematic conduct, along with dates and court disposition. This information must be provided in English. Your signature here gives Indiana University permission to access pertinent criminal records.

If additional legal charges or injurious behaviors as described above occur prior to matriculation at IU, you must provide updated information to the CAD Committee, c/o Office of Admissions, 300 N Jordan Avenue, Bloomington, IN 47405, before attending classes. The committee must review the new information prior to your matriculation at IU.

I certify that I have read the instructions above and the information provided on this form is complete and accurate.
